Insurance Info

Need help figuring out Insurance? We got you covered.

Insurance / Payment Information

Medical Necessity

Medical plans generally cover services they determine to be medically necessary. Policies differ by carrier; your physician’s recommendation may not always align with your plan’s coverage rules. We can estimate costs if you prefer to self-pay for a service that isn’t covered.

Authorizations / Referrals

Check your policy for any pre-authorization or referral requirements before seeing a specialist. Without required approvals, you may be responsible for the visit or need to reschedule.

Patient Responsibility

Self-pay patients and those with co-pays are expected to pay on the day of service. If you’re unable to do so, your appointment may need to be rescheduled.

Insurance Carriers Accepted

Due to narrow networks and policy differences, always confirm with your plan that we are in-network. If you don’t see your plan listed, we may still accept it—please call to ask about your specific coverage.
